Default templates: what new practitioners get vs what they actually use
Every new workspace gets a set of default templates based on the profession chosen at signup. We now have ten weeks of real usage data linking notes to templates, and it shows the defaults are tuned for paperwork while practitioners spend their days writing notes. This page sets out the gap and a concrete change spec to close it.

What the usage data says
Five findings, from 251,618 notes across 4,932 active workspaces.
1SOAP is the universal note, and we don’t always provide it
SOAP ranks #1 or #2 by usage in all seven analyzed profession groups. DAP is the clear second pattern, top 20 in six of seven groups. Yet five professions get no SOAP template in their defaults, and fourteen get no DAP.
Professions whose defaults include no SOAP note. In usage, coaching’s group has SOAP at #1 across 133 workspaces; social work sits in mental health where SOAP leads with 607.
2Defaults are paperwork, usage is notes
Personal Training ships 18 templates and not one is a note. Coaching ships 26 with a single note. Production runs the other way: the most‑used templates in every analyzed group are daily documentation notes such as SOAP and DAP, wrapped by a thin layer of intake and consent forms.

3Psychiatry has the weakest set in the file
14 templates, no SOAP, no DAP, and its only assessment is a lab slip. The psychiatry‑specific follow‑up notes cover the progress‑note job, but GAD‑7 and PHQ‑9 sit at #6 and #7 in mental health usage, in roughly 345 workspaces each, and every other mental health profession gets them by default. Psychiatry does not.
4Allied health wants discipline‑specific SOAP
The allied health top 20 is full of self‑built OT, PT and speech SOAP variants. One OT’s customized SOAP logged 1,261 notes on its own. Acupuncture and chiropractic already get discipline SOAPs in their defaults; OT, PT and SLP get the generic one or nothing. Practitioners are building what we could be shipping.
5Some near‑universal defaults never chart in usage
Three testimonial release forms ship to 19 of 20 professions and a minor software‑use consent ships to 18. None of them appear in any group’s top 20 during the ten‑week window. Spanish variants ship inconsistently: a Spanish release form goes to personal trainers and massage therapists, while Psychiatry gets no Spanish forms at all. Meanwhile 93 of the 179 distinct templates ship to exactly one profession.
One caveat, so we don’t overclaim: one‑time forms naturally log fewer notes than daily notes, so absence from a top 20 is a weaker signal for consent forms than for notes. But privacy notices and card authorizations still chart in every group; these never do. Treat the removals as a strong hypothesis, not a proven fact.
Where this fits
Two horizons are in play. A larger rethink of the whole new‑customer template experience is being pitched separately: a curated starter set of roughly ten templates, a guided first visit to the Templates page, and an import path for switchers. This page is the near‑term piece. Most of it is a pure config change to the existing per‑profession sets; the only content work is three new SOAP variants and two audits. The two are compatible; shipping this first also makes the defaults a cleaner baseline for the redesign.

Six moves, ordered by confidence in the data.
1. Ship a universal notes core
Add SOAP Note, DAP Note and a plain Progress Note to every profession missing that pattern. This is the single highest‑confidence change: all seven analyzed groups converge on exactly these note types. Applying it to every signup profession is our recommendation drawn from those group rankings, not a per‑profession measurement.

2. Fix the Psychiatry set
Psychiatry’s SOAP and DAP arrive with move 1. On top of that, add GAD‑7 and PHQ‑9, which every other mental health profession already gets.

Three new templates to build. Practitioners hand‑roll these at scale, so the raw material for the content spec exists in production: parse the top custom variants for what they converge on (Rohman has the follow‑up query ready), then run a clinical content review before publishing.

Remove the three testimonial release forms and the minor software‑use consent from the default sets. They stay in the template library for anyone who wants them. Every unused default is noise between a new practitioner and the template they need on day one.

5. Tidy the Spanish variants
Stop shipping scattered Spanish one‑offs in the defaults. Before removing, confirm the Spanish forms are discoverable in the library so nobody loses access. If Spanish defaults matter, do them properly as a language preference, not three random forms.

6. Two follow‑ups for the team
First, audit that every default note template carries a signature field: 51 to 74% of real form‑notes have one in every group, and it feeds the sign‑and‑complete work. Second, a template titled “SOAP Note - don’t publish yet” is live in 18 wellness workspaces with 310 uses. It is not in the current defaults config, so it appears to be a leaked default from an earlier set. Worth tracking down and renaming or retiring.

The ask
Approve now: moves 1, 2, 4 and 5 as a single config change to the default sets. Growth hands engineering the machine‑readable spec below; no product surface changes.
Follow up: move 3 (three SOAP variants) gated on the custom‑variant content parse and a clinical review, plus the two audits in move 6.
Measure: compare template usage per active workspace for post‑change signups against the current baseline via the note‑to‑template link, once the next cohort has a comparable window of data.

Implementation notes: remove by TemplateId, not name. The current config has a bug we found while building this: in Massage therapy and Nursing, the IDs for “Testimonial Release Form - Colleagues” and “Third Party Financial Responsibility Form” are swapped relative to the other 17 professions, so name‑based removal would cut the wrong form. The machine‑readable spec (JSON) carries the verified per‑profession ID for every removal.

Read before quoting
The window is short. The note‑to‑template link only exists since about 5 May 2026, so usage covers roughly ten weeks (5 May to 13 Jul). Fine for rankings; do not annualize the counts.
Coverage is partial and group‑level. 35% of registered workspaces have no profession set, and the unknown buckets hold 23% of note volume. Rankings are computed per profession group, not per signup profession, so the per‑profession spec is an inference from group behavior. Directional, not census‑grade.
Definitions. An active workspace is one with at least one template‑linked note in the window; rankings count notes per template within each group. Analysis by Tim, five read‑only production queries, 13 Jul 2026.
Defaults are the mechanism. Every top‑20 row in the usage data is a workspace‑saved copy, and defaults get saved into workspaces at signup. That is exactly why getting the default sets right matters: they are how library content reaches volume.
Removal candidates are a judgment call. The additions are backed directly by usage rankings. The removals rest on absence from every top 20, which is a weaker signal for one‑time forms. Everything removed stays available in the library.
